The Scott Center for Observation Treatment and Transition and the Kidney Foundation of Northwest Ohio are pleased to announce the launch of a partnership that includes shared office space and a collaborative co-working model to address health disparities in our community. The Scott Center assists families and individuals with sickle cell disease in transitioning from pediatric to adult care and works to minimize infant mortality in communities of color. This community-based organization bridges the gap between medical professionals and patients by advocating on a medical, educational, or familial level. The Kidney Foundation of Northwest Ohio is a leading supporter of people with end stage renal disease, providing assistance with medication, transportation and nutrition supplements through transplantation. Because communities of color are 3x more likely to suffer from end stage renal disease, the organization is committed to working with other non-profits to provide education and prevention services.
